Ensure you’ve included Rx Forms with our digital versions to ensure accuracy and efficiency. Print or save for later.
Step Three - Send or Schedule a Pickup
If you’re located within the Greater Houston, Texas area, schedule a Local Pick-Up. If you’re located outside this area, please print a complimentary shipping label.
You’re in the neighborhood of our laboratory, which qualifies you for free pickup and delivery service.
Your practice falls outside our driver’s local pick-up area, please print out a complimentary USPS shipping label.
Step Two – Case Scheduler
Schedule with confidence with our easy-to-use case scheduling calendar. To receive your estimated turn time for your restoration simply select the product and date that the case will be shipped from your practice.
